About Grand Junction and the Surrounding Area

Grand Junction is the largest community between Denver and Salt Lake City and the center of business, professional and cultural activities for an extensive geographic area. As the result of being a regional "hub", Grand Junction possesses the amenities and opportunities of a city several times larger.

Traditionally an area of agriculture, energy and tourism, Grand Junction has attracted new services and light manufacturing, helping to broaden the economic base. The community offers a strong public school system, a four-year college with post-graduate programs, a symphony orchestra, regional arts center, museums, major medical facilities, an extensive parks and recreation program, golf, tennis and athletic facilities.

The Grand Junction area enjoys four distinct seasons, all generally mild in nature. Sunny, dry weather is abundant, allowing residents to take advantage of the numerous outdoor recreational opportunities available in western Colorado and eastern Utah.

The resort areas of Aspen, Vail, Beaver Creek, Telluride and Powderhorn are all easily accessible to enjoy winter and summer sports, cultural events and festivals. Close to the Grand Junction area are some of the finest mountain and canyon areas in the Rocky Mountain region ideal for skiing, hiking, camping, hunting, fishing, road and mountain bicycling and whitewater river rafting.

Quality Project Manager
QHN is hiring a self-motivated Project Manager with experience and a willingness to lead projects involving implementing technology solutions and tools that improve the health of the communities we serve. This role is a combination of project management as well as liaison with our stakeholders. The successful candidate will work with practices, agencies and stakeholders to support quality work and state projects such as: The State Innovation Model (SIM), Patient Centered Medical Home, Comprehensive Primary Care Plus (CPC+) and the Qualified Clinical Data Registry (QCDR) as well as ensuring deliverables will fall within the applicable scope and budget. We are looking for a creative and dedicated individual who will fit with our collaborative culture. Quality Measurement Data Analyst
QHN is hiring a knowledgeable quality measurement data analyst. The successful candidate will have an understanding of Clinical Quality Measures and will have the technical expertise to ensure the quality and accuracy of data, then process, design and present it in ways to drive value to stakeholders. Some responsibilities include:
  • Lead the QHN Quality Measurement reporting program
  • Work with various organizations on the acquisition, validation and reporting of needs for each business segment
  • Recommend and establish new quality measurement certification programs
  • Training end users on new reports and dashboards.
Position Requirements
Formal Education & Experience:
  • Bachelor’s degree from an accredited university or college in computer science or related field. Will consider relevant job experience.
  • Ability to perform several tasks concurrently with ease and professionalism
  • Work experience as a data analyst or in related field and experience working and preferred to have with quality measurement initiatives such as: eCQM, HEDIS, P4P, value based payment models, advanced payment models, etc.
  • Ability to work with stakeholders to assess potential risks.
  • Ability to analyze existing technology solutions and provide software solution recommendations.
  • Ability to translate business requirements into non-technical, lay terms.
  • Ability to interface with a variety of clinical and non-clinical agencies, assisting with data retrieval.

Grant and Communications Coordinator
QHN is hiring a self-motivated individual that excels in communication of all kinds. The Grant and Communications Coordinator is a role that primarily focuses on grant writing and reporting, with the secondary responsibilities of supporting the QHN marketing message and developing educational materials. The successful candidate will have experience in grant writing and/or communications. Responsibilities include:
  • Grant Management: preparing proposals; identifying and responding to public and private grant opportunities; writing grant activity reports for government, corporate and other funders.
  • Communications: support marketing firm; create educational materials, maintain library for press coverage.

Formal Education & Experience:
  • Preferred bachelor’s degree in communications, English, journalism or similar field. Will consider applicants with relevant experience
  • Excellent grammar and strong research skills

Client Relationship Manager
QHN is looking for a Client Relationship Manager. The Client Relationship Manager is a key role in our Business Development Team. The goal for this role is to support the QHN participants as a trusted and loyal advisor, assisting our partners to maximize the efficacy of systems involved in health outcomes. Responsibilities include:
  • Provide consultation, education and direction to Western Colorado organizations and providers to support the adoption of health information technology, health information exchange and promote innovative uses of electronic health information. The areas of focus are on practice transformation, providing support for the transition to value-based payment models, and improving care coordination.
  • Conducting business reviews to ensure clients are satisfied with their products and services.
  • Monitoring company performance against service level agreements and flagging potential issues.
  • Has knowledge of Meaningful Use, MACRA, MIPS, and other reporting which may be needed, to assist organizations through our rapidly changing business world.
