QHN Participation

QHN gives you fast access to accurate patient information - from testing results to longitudinal health records (results over-time) and secure email - to improve patient care, office efficiency and care coordination.

Our commitment to healthy communities also means providing users with other tools to improve the quality of care, including high value HIE applications, support for practice transformation activities and achieving Meaningful Use milestones.

How to Participate

We are happy to assist you in the process of becoming a QHN participant and are pleased you have an interest in joining this powerful, progressive regional network of medical providers.

The first step in joining QHN is to take a moment to contact our Customer Support team and they will refer you to one of our Clinical Account Managers who will provide you with personal assistance to complete the enrollment process. You will be contacted by a Clinical Account Manager to set up an appointment to discuss the services that QHN provides, what EHR your organization utilizes and what your data and connectivity needs are so we can begin to assess the best fit for your organization.

QHN is committed to helping our medical partners and information providers comply with evolving policies and operating procedures, while safely and securely exchanging patient data. For more information you may review a copy of the QHN data use policies here.

New QHN participants will be requested to complete the below forms:
  • Electronic Commerce Agreement (eCommerce Agreement)
  • QHN New User Packet, which includes: Practice Profile Form – This form is completed by the designated organizational contact. Contact should fill-in all designated fields and acquire appropriate signatures and the QHN New User Form – This form is to be completed by the individual(s) from the participating organization requesting access to the QHN system. The User is required to complete all designated fields, initial, sign, and date the form.

Education and Training Support

We look forward to helping you make a smooth transition as you add access to QHN’s longitudinal health record (Patient Summary), QHN Direct, Alerts or other exchange services to your daily workflow. We are happy to provide training for physicians and care team members on using the QHN system, including detailed instructions on how to access the information in the extensive QHN data repository. You’ll also learn about QHN policies, security measures and user guidelines.
