Connecting medical care across a large network in western Colorado was just the beginning.
We knew we had to do more to help whole-person care. Often up to 80% of factors that make up a person’s whole health are related to Social Determinants of Health or SDoH such as housing, food insecurity, loneliness, transportation, and others. As our community focused on SDoH challenges, we started the Community Resource Network (CRN). This award-winning, team-based service coordination platform integrates with our western Colorado Health Information Exchange (HIE) to give providers a whole-person picture of health including medical, behavioral, and social.
Today, CRN connects nearly 200 Partners to over 2500 resources. Partners can find resources, send referrals, join virtual care teams, and communicate with other care team members around their clients. This helps providers focus on whole-person wellness to improve outcomes and prevent costly healthcare interventions. Using CRN, care organizations in western Colorado have helped over 31,000 clients and sent over 15,000 referrals.
Recently we launched CollaboRise, a convener service that helps communities and organizations navigate complicated issues and processes surrounding whole-person health and SDoH. Today we convene, facilitate, and coach communities and organizations, empowering lasting, positive change in their communities.
And, as of May 2024, QHN has affiliated with Contexture to create one unified HIE platform serving millions of patients across Colorado and Arizona. Together, we will continue to connect people for better health well into the future.
