QHN is proud to participate in the Patient Centered Data HomeTM (PCDH) project. This initiative allows us to exchange data with other HIEs and for providers to access in real-time patient information on care events that occur outside of the QHN service area. As part of the Patient Centered Data HomeTM (PCDH) initiative QHN is now live with HIE-to-HIE data exchange with Colorado Regional Health information Organization (CORHIO), the Utah Health Information Network (UHIN) and Arizona Health-e Connection (AzHeC). Providers can access data from these sources by initiating a simple External Document search through the QHN system.
