The pandemic has not only highlighted the importance of community members working together, but also how collaboration can drive community-centered health outcome improvement.
Telligen QI Connect, a national Health Quality Improvement organization that QHN partners with, has recognized us as a Community Quality Champion and awarded us with the 2020 Community Quality Champion Award.
For more than four decades, Telligen has assisted multi-sector partners with improving community-wide health and wellness. They are recognizing QHN’s hard work, passion, and dedication to helping our participants provide patients and clients with the highest quality of care.
As western Colorado’s Whole Health Information Exchange, QHN helps medical and behavioral health providers in western Colorado securely share patient data that enhances care coordination, reduces duplication of services, and identifies individuals at risk so that efforts can be focused where they are needed most. In addition, our Community Resource Network is a team-based service coordination platform that boosts community-wide health and makes for easy integration of crucial Social Determinants of Health data.
